1.  Application below
2.  Proof of your Purple Heart in the form of either a DD214 or the orders for your award
3.  A current picture of yourself

We ask you to provide photo documentation to facilitate our link up and identify you in an airport. Your privacy is extremely important to us; we do not sell or provide your information to third party.
